Athletic Forms
The DSU Athletic physicals and forms will now be completed once you arrive here at DSU. You will not be responsible for printing off the forms and getting a physical before you arrive. The physical will be done right here at DSU as part of your first days activities. All you need to bring is a copy of your insurance card and $40.00 cash for the physical.
Athlete Information Check List
Here is a check list of information that will need to completed before participation.
- Photocopy of both the front and back of insurance card (Note: All Athletes’ Insurance Coverage must be acceptable in South Dakota for the full academic year.)
- Student Athlete Information Packet
- HIPAA (Health Information Portability and Accountability Act) Form
- Athlete Health Participation/Physical Exam Form
- You must read the DSU Student Athlete Handbook
